Neck Herniation and Neck Pain in Desk Workers

Every 2.5 cm your head moves forward roughly doubles the load your neck muscles carry. The real cause of neck pain in office workers — and what helps.

An adult head weighs about 5 kilograms. For every 2.5 cm the head drifts forward, the force the neck muscles must produce to balance it roughly doubles. In a neck stretched towards a screen, that load can climb towards 20 kilograms — and it lasts eight hours a day.

How do the symptoms start?

Complaints usually begin as tightness that appears towards the end of the day. Over time the picture changes:

  • Constant tension between the base of the skull and the shoulders
  • Restriction when turning the head
  • Headache starting at the base of the skull and spreading over the back of the head
  • Pain or numbness travelling from the shoulder into the arm, sometimes to the fingers
  • A feeling of weakness in the arm

The last two suggest a nerve root is involved and warrant assessment.

What does "loss of the neck curve" mean?

The cervical spine normally carries a gentle curve. Prolonged forward posture can reduce that curve, and reports describe it as straightening of the cervical lordosis. On its own it is not a disease; it also appears in people without symptoms. What matters is not the imaging finding but the load-bearing capacity of the neck.

Ergonomics: small adjustments, large difference

  • The top edge of the screen should be at eye level; a laptop must be raised
  • Elbows at roughly 90 degrees, forearms supported
  • Feet flat on the floor or on a footrest
  • Avoid holding a phone between shoulder and ear
  • A short standing break every 30–40 minutes

Ergonomics alone is not enough. Even the best desk setup does not undo the effect of hours spent motionless.

Why is exercise essential?

Exercise carries the strongest level of evidence in neck pain. The programme has two components:

Activating the deep neck flexors. The small stabilising muscles deep in the neck shut down in the presence of pain and need to be switched back on.

Shoulder girdle and upper back strength. The neck does not settle until the muscles that stabilise the shoulder blades are strong.

When should you seek help?
