One-to-One Physiotherapy Versus Group Exercise

The same exercise can produce different results in two people. What makes the difference is not the name of the movement but how it is adapted to you.

A list of exercises can be found online. What cannot be found is that list adapted to you. This is where one-to-one work begins to differ.

Same exercise, different body

Think of a squat. Hip structure, ankle range of motion, a past knee injury and trunk control each change how the movement should be performed. In a group setting those differences cannot be seen; everyone receives the same instruction.

The result is that a movement which is ideal for one person becomes the source of pain for another.

Technique correction has to be immediate

A faulty movement pattern is reinforced every time it is repeated. There is a large difference between a fault being spotted and corrected within the session and being noticed weeks later. In one-to-one work every repetition is watched.

The dose is set for the person

Even when two people follow the same programme, the load they carry is not the same. A set that one finds easy is beyond the limit for the other. In one-to-one work sets, repetitions and rest are adjusted to the state of that day — not to a ready-made template.

The difference grows when there is pain

Where a painful area is involved, deciding which movement to adapt and which to remove temporarily is a clinical judgement. That judgement requires knowing the person's history and examination findings.

Measurement and follow-up

A less discussed but decisive aspect of one-to-one work is follow-up. Strength, range of motion and balance measurements taken at the start are repeated at intervals. This means:

  • It becomes visible which area is lagging behind
  • The programme is updated on the basis of data
  • Progress rests on a number rather than on "I feel good"

  • Anyone with a previous injury
  • Anyone with ongoing pain
  • Anyone in the period after surgery
  • Anyone restarting after a long inactive period
  • Athletes with a specific performance goal
